SoulWays Intake form
Instructions:
1) To fill out the form on your computer, you need to have Adobe Reader installed.
To fill out the form on your phone, you need to have an app installed, like: Adobe Fill & Sign.
2) Click "download file" link below, and then click "save file" to download the intake form to your computer or phone.
3) Once the form is downloaded to your device, open it with Adobe Reader or Adobe Fill & Sign.
4) When the downloaded form is open on your screen, click on each blue text box to type your responses.
5) When you're done entering your information, BE SURE TO 'SAVE' THE FILLED OUT FORM on your computer or phone.
6) To get the saved filled out form back to me, please attach it to an email and send it to: [email protected]
